Responsibilities:
A Patient Care Assistant (PCA) is a Certified Nurse’s Aide (CNA) who under the direction of the Primary Care Nurse assists clients in their homes by providing personal health services, combined with selected household activities.
The PCA/CNA assists clients with supportive services necessary to provide and maintain physical and emotional comfort in the client’s home environment. The PCA/CNA assists clients toward independent, safe and healthful living.
Responsibilities and Duties:
The PCA/CNA will follow the written instructions of the primary care nurse in accordance with physician’s orders.
Assists the client with personal care activities including bathing, oral hygiene, feeding and dressing, and encourages client with self-motivation.
Assists the client with exercises, ambulation, transfer activities and reminds the client regarding their medication schedule.
Assists to promote client’s mental health through conversation, reading, games, and other activities not tiring to the client.
Informs primary care nurse of any change in client’s condition or family situation.
Maintains and submits, according to agency policy required time slips.
When permitted, accompanies client outside the home either walking or in a car driven by someone else.
Works in a team environment and independently without direct supervision.
