Job Summary:
Daytime supervisor for a 65 bed skilled nursing facility. Noble Horizons is a non-profit continuum of care senior living community located on 110 pristine acres in Salisbury CT. Noble Horiozons offers the highest levels of skilled nursing care. The position is a 40 hour, Monday thru Friday assignment with responsible supervision for our skilled care unit only.
Performance Requirements:
Responsibilities:
Knowledge of the resident's condition at all times. Provides nursing care according to the physician's orders and in conformance with recognized nursing technique, procedure and administration policies. Recognizes and interprets symptoms, reports resident condition, assists with remedial measures and notes changes in resident's condition. Maintains accurate and complete records of nursing observation and care utilizing paper and EHR. Assists in teaching residents good health habits. Must be able to assume the responsibilities of a Charge Nurse when necessary.
Physical Demands:
Stands, walks short distances, stoops, assists with lifting and transferring residents. Must be in good physical and emotional health and be emotionally stable and mature.
Special Demands:
Must demonstrate understanding and tact in dealing with the elderly person, ill patient, visitors and family members. Demonstrates alertness and skill in recognizing and identifying symptoms and use initiative and sound judgment in emergency situations. Exposure to physically and verbally disruptive residents and physically demanding resident care. May cover as a Charge Nurse as deligated by the Director of Nursing.
Qualifications:
Education And Training:
Current licensure in the State of Connecticut. Graduate study and / or experience in geriatric nursing desirable but not necessary. Computer skills required.
Experience:
Experience in geriatric nursing preferred but not required.
Job Knowledge:
Knowledge of general nursing theory and practices and their application for better geriatric patient care. Thorough knowledge of methods and techniques involved in performing general nursing services. Leadership qualities necessary. Computer skills necessary.
Job Relationship:
Source: Nurses registered in the State of Connecticut. The local community within reasonable driving distance as traveling in inclement weather will be necessary.

Workers Supervised:
Charge Nurse, Nurse Assistants and Auxiliary personnel when their department heads are off duty.
Job Requirement:
Attends 50% of staff meetings during the year.
Essential Duties:
2. Promotes and participates in an interdisciplinary approach to resident care.
3. Oversees and assists in the coordination of resident care by certified Nursing Assistants.
4. Promotes and participates in educational opportunities, both formally (at least 1 hour per month) and informally.
8. Promotes and supports the philosophy, goals and objectives of the Nursing Department.
9. Regularly attends unit and staff meetings. (At least 50%).
10. Conducts daily rounds on assigned unit to assure that safe and effective quality care is provided.
11. Delivers care according to established organizational policies and procedures.
12. Initiates counseling and disciplinary actions of staff assigned to the clinical unit.
13. Serves as a positive role model for other members of the health care team.
15. Assists in the planning and implementation of creative and innovative projects and approaches to resident care at the unit level.
16. Provides care as outlined by physician's orders, nursing judgment and interdisciplinary care plan (i.e., medications, therapies, treatments, etc.)
17. Administers medications and treatments in a timely and safe manner.
18. Assesses residents' conditions on an ongoing basis, accurately and promptly reporting significant changes to supervisor, family and physician.
19. Keeps accurate and timely documentation reflecting resident's care and condition in accordance with State and Federal regulations.
20. Performs duties in a manner that assures the highest level of quality care and quality of life for all residents.
21. Effectively communicates with residents, families and fellow co-workers.
22. Makes independent clinical decisions regarding residents' need for medical or nursing care and communicates same to appropriate personnel.
23. Takes responsibility for evaluation and investigation of Cobble / Cottager status, communicating problems for follow-up and further problem-solving.
28. Ability to review residents admitted and compare physician orders with each resident's plan of care to assure appropriate implementation of physician's care plan.
29. demonstrated proficiency in the EHR and utilizes it accordingly.
Working Environment:
Works in a clean, well lighted, well ventilated modern nursing home. Work environment may be noisy, have frequent interruptions and high stress level. May be exposed to blood, body fluids and infection.
